The 2026 AHA/ACC/ESC/WHF Expert Consensus Document: Second Universal Definition of Heart Failure puts new weight on Stage A and Stage B. The stages before symptoms appear, where prevention still works. It also names #AI-enabled #ECG among the tools worth watching in this window. That's the opportunity. Proving it in practice, across populations and care settings, is the work ahead for all of us building in this space. A flag alone doesn't save anyone. Someone has to route that patient to an echo. A clinician has to trust the signal enough to act on it. At Simplex Quantum, we think about this constantly. Not just where AI-enabled ECG detects risk. Medication alone doesn't drive long-term outcomes in obesity care. Nutrition, body composition, remote monitoring: continuous data matters as much as the drug itself. Rather than building every solution itself, Novo Nordisk is investing in a partner ecosystem that makes it easier to integrate specialized digital health tools into patient care. This approach could extend beyond obesity. In heart failure care, no single digital solution can meet every patient's needs. We're already seeing platforms that help clinicians discover and evaluate digital health tools based on specific use cases. The next question is whether the same ecosystem model will expand to patient-facing digital health. The future of healthcare AI may not be about building every solution. The mechanism is well understood. Emotional stress triggers a catecholamine surge. Heart rate climbs. Blood becomes more prone to clotting. For people with underlying risk factors, that's a combination worth monitoring. What's different in 2026 is the infrastructure. ⌚Wearables, RPM platforms, and continuous monitoring tools are now part of daily life for millions of patients. Heart rate, rhythm, oxygen saturation, activity patterns that once required a clinic visit is now available in real time. That opens up new possibilities during high-stress events like this one. If a device flags something unusual during a tense match, it's worth paying attention. Not every alert is an emergency. For the first time, four major societies jointly established that obesity, type 2 diabetes, chronic kidney disease, and cardiovascular disease form a single interconnected syndrome requiring unified management. The guideline outlines 10 recommendations. Reading through them, what stands out is how many depend on doing more with existing clinical data. Routine CKM staging across all adults. Cardiovascular risk quantified using the PREVENT equations, not estimated informally. Systematic screening for metabolic and kidney disease. Routine assessment of social determinants of health as part of clinical evaluation. That's where technology comes in. Embedding risk stratification into existing workflows. Extending screening beyond specialty centers. Identifying patients quietly accumulating cardiometabolic risk before symptoms appear, before the ER visit, before the readmission cycle begins. A research group at Osaka University presented findings at last month's 69th Annual Meeting of the Japan Diabetes Society that made us rethink how we frame obesity and diabetes prevention. Among abdominally obese individuals, preferences for high-fat and high-carbohydrate foods were directly linked to actual intake. That part isn't surprising. What is striking: those same food preferences were significantly associated with social factors including long working hours, late-night sleep patterns, low income, and lower education levels. In other words, the desire to eat certain foods may itself be shaped by the social environment a person lives in. This matters clinically. If the craving itself is socially determined, interventions aimed purely at individual behavior change will always hit a structural ceiling. Telling someone to eat more vegetables when they work 60-hour weeks and sleep past midnight isn't a treatment plan. It's a wish. Japan already conducts annual health checkups at scale. The infrastructure exists. #DigitalHealth and #AI cannot fix the social conditions that drive risk, but they can help identify who is accumulating that risk quietly, before it becomes irreversible disease. Research show men are 33% less likely to visit a doctor than women, and typically only go when something is clearly wrong. The problem with heart failure: nothing feels clearly wrong, until it does. Fatigue, mild shortness of breath, slight swelling in the legs, early symptoms are easy to dismiss as aging or a busy life. By the time most people seek care, the disease has already been progressing silently. 38% of new heart failure cases are first found in the emergency room or during a hospital stay. At that point, 1-year mortality exceeds 30%. Earlier detection may change that outcome. A 30-second single-lead #ECG, the same signal already used in many consumer wearables, can now detect heart failure risk before symptoms become impossible to ignore. But we now know the trajectory can be more dynamic. With treatment, some patients improve; their functional capacity recovers, their phenotype shifts. The category of HFimpEF exists precisely because of this. Yet the dominant framework still relies on a single LVEF measurement at a single point in time. One number. One moment. Last year, HFSA, ESC, and the Japanese Heart Failure Society addressed this in a joint consensus statement, calling for classification based on LVEF trajectories over time, rather than static cutoffs. Japan's updated clinical guidelines, revised in 2025, reflect the same direction, formally introducing HFimpEF as a distinct category. The clinical implication is important. A patient's management should follow where their heart function is going, not just where it stood at one visit. That observation is part of what led us to build NIHA-HF. It's a significant moment. And it points to a bigger problem the field has been slow to confront. Both HFmrEF and #HFpEF share one defining feature: ejection fraction that looks "normal" on a basic screen. Together, they account for the majority of heart failure patients. Yet tools built around low EF detection are, by design, blind to both. This isn't going to get easier. Aging populations and the rise of hypertension, diabetes, and obesity are driving these phenotypes higher. The share of heart failure that EF-based tools can't see will only grow. The HFSA statement challenges the binary way heart failure has long been classified. The tools we build for it should follow. At Simplex Quantum, we've been working on a different approach — assessing functional severity from a single-lead ECG, rather than relying on EF cutoffs alone.
